By signing and submitting this form you officially notify Diamond Lake Productions of your successful media upload and formally request payment as defined by your Work Order Agreement and disbursed by your preference provided in your Payment Method Authorization form.

Payments are disbursed NET-30 to the submission of this form.

Media submissions beginning later than 48 hours following the assignment (and/or not meeting the expectations of the assignment) may subject to a reduction of payment as defined in your Work Order Agreement.

As a contractor, all wages disbursed will be counted in your 1099 issued each year in reference to your W9. If any information is no longer accurate to what was submitted at the time of your on-boarding, please notify Andy (andy@diamondlakeproductions.com) right away.
